MIGATE COLLECTION STATEMENT
Personal Information - is any information, whether recorded by document or electronically, about you.
It includes opinions about you and your work performance and may vary from sensitive (eg. medical records) to routine (for example your address and telephone number).
Sensitive Information - is a special category of personal information including details or opinions about you;
Racial or ethnic origin; membership of a professional association, trade association or trade union; Criminal record; Health or disability (past or present); and Expressed wishes about future provision of health services.
Who Will Be Collecting Your Personal and Sensitive Information at MIGATE
MIGATE's employees will collect your personal and sensitive information for its own use in connection with your work placements and employment by MIGATE.
If you wish to contact MIGATE about your personal or sensitive information, please contact the MIGATE Privacy Officer at the MIGATE office where your registration and employee records are held.
How MIGATE Collects Your Information
You fill out and submit a registration form (including forms available on MIGATE's website); You submit a vacancy details form, MIGATE receives any reference about you; MIGATE receives results of enquiries that MIGATE may undertake of your former employers, work colleagues, professional associations or registration bodies; MIGATE receives the results of any aptitude, competency or medical test; MIGATE receives performance feedback; MIGATE receives any complaints about you; MIGATE receives any information about a workplace accident or incident in which you are involved; MIGATE receives information about any insurance investigations, litigation, registration or professional disciplinary matter, criminal matter, inquest or inquiry in which you are involved; MIGATE receives any other information about you from you or any other source.
Storage of Information By MIGATE
All personal information that MIGATE holds will be stored by hard copy and /or electronic filing. All information stored electronically will be stored within secure software accessible only by or through authorised MIGATE personnel.
How Your Information Will Be Used By MIGATE
Your personal and sensitive information may be used in relation to:
Your actual or possible work placement/employment; your performance appraisals; our assessment of your ongoing performance and prospects; Any test or assessments that you may be required to undergo (whether medical, academic, aptitude test, or otherwise); Our identification of your training needs; Workplace Rehabilitation; Management of any complaints, investigation or inquiry in which you are involved; Any insurance claim or proposal that requires disclosure of your personal and sensitive information.
Accuracy of Personal Information
You should ensure that the personal and sensitive information that you provide to MIGATE is accurate, complete and up to date.
Disclosure of Your Information to Others
Your personal and sensitive information, in most cases, may only be disclosed with your consent to:
Potential and actual employers/ host trainers and clients of MIGATE; your referee; MIGATE insurers; A professional association or registration body that has a proper interest in the disclosure of your personal and sensitive information; A workers compensation body; enrolments to Tafe: MIGATE Contractors and Suppliers; and any person with a lawful entitlement to obtain the information.
Failure to Provide Information to MIGATE
If you do not provide the information MIGATE requests from you, MIGATE may be unable or limited in its ability to place you in work.
Assessing and Correcting Your Information with MIGATE
Subject to some exceptions, which are set out in the National Privacy Principles (Principle 6 Access and Correction), you have a right to access and have a copy of the information about you that MIGATE holds.
If you wish to exercise your rights of access and correction, you should contact the MIGATE Privacy Officer at the MIGATE office.
In some cases, MIGATE may impose a moderate charge for providing access to your information.
